Barnes and Noble has a nice way to encourage kids to read and a handy form to record what they read. We have done this program before- There is a downloadable kit with fun activities and a reading journal to record books that have been read. There is a suggested grade level reading list, too. (The kit includes the reading journal and there is a Spanish version)
